A server
I used a VPS from DirectSpace Networks, buget VPS hosting starts at $4/month. FranTech has some budget VPS that start at only $15/year when they are available that are also fun. I have had trouble with DirectSpace not recording my paypal subscription for 2 months in a row and am currently suspended. I no longer recommend them!
A phone number.
I used IPKall which provides free DIDs.
~~I am assuming that IPKall supports a maximum of a 2 hour phone call... because that's the longest anyone has called for.~~ My assumption was wrong, the longest call so far has been for a single 238 minute call!

I created a script to setup a basic Asterisk/FreePBX server on a virgin CentOS install
Perl and the Asterisk::AGI module
I used cpan and ran install Bundle::CPAN install Asterisk::AGI install DBD::mysql
the pi agi script that I wrote
... and pi!
